Employee Licensing

Licensing Types

Gaming:

All Employees working or being hired to work in a gaming position, are required to go through the gaming license process in order to work on the gaming floor.


The Gaming License are currently broken down into three categories:

  • Class II
  • Class III
  • Class II&III


Gaming License Fees:

  • All employees are currently assessed a mandatory $50.00 bi-annual administrative fee when they initially apply for their gaming license and at every License review. 

Non-Gaming:

Currently do not require a gaming license

New Hire Licensing Process

Prior to starting the licensing process, the Applicant will generally start by being offered a position with a department at the respective gaming facility. Once offered a position, they will sign an Intent to hire form. Once this form is received by the Licensing Investigator the investigator will begin the licensing process.


  • An Applicant who has never held a gaming license with the Ho-Chunk Nation, will be required to complete the licensing requirements for a new hire gaming license application.


  • If you are a rehire who previously held a gaming license with the Ho-Chunk Nation, please inquire with the gaming facility’s onsite Licensing Investigator for the necessary requirements for your gaming license application.


         The Applicant is responsible to:

  • Complete their gaming license application, completely and honestly.
  • Schedule an appointment to get their finger print(s) scheduled, show up to the appointment and submit their finger prints.
  • Sign both the wage assignment and Release forms.
  • Schedule an appointment with the on-site Licensing Investigator to go over the submitted application.
  • Provide all required/requested documentation as indicated on the gaming license application and/or requested by the Licensing Investigator.


5. The Applicant will be given instructions on how to access their account and how to complete each of the four items noted above.


6. It is the responsibility of the applicant to provide a truthful answer to all questions on the gaming license application.

Current Gaming Employee Review Process

1. The Applicant will receive an automated email generated by the licensing software program 60 calendar days from the review by date of the applicant’s current gaming license.


2. It is the responsibility of the applicant to provide a truthful answer to all questions on the gaming license application. 


         The Applicant is responsible to:

  • Complete and submit their gaming license review application, completely and honestly.
  • Sign both the wage assignment and release form.
  • Schedule an appointment with the on-site Licensing Investigator to go over the completed application.
  • Provide all required/requested documentation as indicated on the gaming license application and/or requested by the Licensing Investigator.


3. It is the responsibility of the applicant to provide a truthful answer to all questions on the gaming license application.

Change of Information

It is the responsibility of the Licensee (gaming employee) to keep the gaming commission updated regarding changes to your personal information and any other information as required to include criminal history.


To do so, log into the licensing software under your account, go to “My PRYME”, under “Resources” Choose the “Change of Information” form, complete it and hit the save and submit button. Please make sure to provide any documentation as needed.


This will be directed to the on-site Licensing Investigator, who will in return update your information into the license software and contact you if they have any questions or require any further information/documents.


The Ho-Chunk Nation uses Merydyan PRYME as their licensing software and no longer accepts paper gaming license applications.

ATTENTION:

  1. Applicant's responsibility: Complete Fingerprint appointment, Create account in Pryme submit Gaming License application, Release form and Wage Assignment. Submit all requested paperwork court/financial if applicable. All information MUST be complete before the applicant is eligible for hire.
  2. A background investigation will be completed on all applicants.

  • Criminal Complaint & Disposition Papers ( civil and/or criminal) 
  • Proof of Fines Paid (if applicable) 
  • Professional License (if applicable) 
  • Passport (if applicable) 
  • Alien Registration or, 
  • Employment Authorization Document (EAD) or, 
  • Certificate of Naturalization Number (if applicable) 

FAMILY, MARITAL, ROOMMATES INFORMATION

  • Current marital I living arrangement status 
  • Spouses/Co-habitant's full name, date of birth, address, home phone number, employer, and occupation, date of marriage. 
  • Previous marriages (if applicable). 
  • Roommate's name, date of birth, employer and occupation. 
  • Children/ Step-Children/ Adopted Children/ Foster Children Full name and date of birth. 
  • RELATIVES/FAMILY MEMBERS 
  • Parents / Step-parents / Legal Guardian / Brothers / Sisters / In­Laws 
  • Be sure to list even if they are deceased
